Who is eligible?

If you have either no dental insurance or if you have Denti-Cal insurance, then you are eligible to be treated in our clinic! If you are uninsured, children pay $40 per visit and adults pay $60 per visit, regardless of the amount of work that is performed. We will do our best to get as much accomplished in each visit. If you have Denti-Cal insurance, we will bill them for the services performed.

How to become a patient

Becoming a patient is easy! Simply give us a call or send us an email and we will work with you to schedule your first appointment. You will need to complete a new patient packet upon arrival in our clinic for your first appointment.

If you're interested in learning more about becoming a patient, email us at Daisy@SGVFDH.org or call (626) 934-2894.

Everything you need to know about visiting our clinic.

Preparing for your visit

Please be sure to bring your photo ID. New patients with Medi-cal will need to provide an ID or social security number to verify eligibility. We accept CASH, DEBIT, and CREDIT for payment.

Please arrive on time. We offer a grace period of 15 minutes, however if you arrive more than 15 minutes past your appointment time, you will be required to reschedule your appointment. If you’re a new patient, we suggest you arrive early to fill out your new patient packet. You must complete these forms in our waiting room before your appointment.

Every visitor is required to wear a mask at all times. View full Covid-19 guidelines here.

Where to enter the clinic

When you arrive at the Hacienda La Puente Adult School, located at 14101 East Nelson Avenue in La Puente, you will want to find the building that has the office located in it, but do not go into the office. Instead, walk to either the side of the building and you will see three doors. Please enter through one of these doors and make your way down the hall to our door that is labeled DENTAL.

What to expect at your visit

When you enter, the receptionist will greet you and take your temperature.  Everyone is required to wear a mask at all times. Our staff will advise you when it is time to remove it for treatment.

New patients will have x-rays taken, following an exam with our dentist.  If there are no signs of heavy build-up on the teeth, then polishing will be done same day.

We will take the time to explain your treatment plan and all options. You are welcome and encouraged to ask questions!

Any deep cleanings/other treatment will be scheduled for another day.
